HeartShare St. Vincent’s Services (HSVS) Nuevo Camino program is dedicated to providing 38 unaccompanied minors, ages 5-12, with a supportive, nurturing environment through transitional foster homes, daily education Monday - Friday, and supportive wrap-around services in the East Brooklyn section of New York City. Our program is rooted in ensuring that service delivery is trauma-informed, culturally appropriate, and tailored to the needs of each child while maintaining all program requirements of ORR.
This Intake Coordinator will interface with ORR on referrals to the program and ensure all documentation is within the portal. This Intake Coordinator maintains frequent contact with case planning staff, to ensure the placement was matched. Keeps track of all vacancies and reflects in the Connections system.
Responsibilities
- Intake of all ORR placements
- Completion of all necessary paperwork and data entry, including but not limited to documentation for intake, replacement, respite, Connections, and Evolve.
- Tracking all youth who enter transitional care and facilitating a quick and smooth by outreaching the placement resource.
- Facilitating the ordering of any items necessary for placement
- Delivering MAPP, and other foster parent training as requested.
- Providing foster parent support through telephone calls, emails, and the drafting and dissemination of the Foster Parent Support Newsletter
- Working collaboratively, and maintaining frequent contact with, the case planning team via email, telephone, and in person
- Participate in required ORR – Nuevo Camino training per the federal and state mandates.
